Verify that you've configured a trigger in the Settings tab matching the correct pipeline and stage. Also confirm the webhook URL is set correctly in your app settings.
This usually means the location token has expired or the app needs to be re-authorized. Try uninstalling and reinstalling the app from the Marketplace. If custom field writebacks fail, confirm the mapped field IDs still exist in your location.
Forms are only created when a webhook fires for a matching trigger. Make sure you've configured at least one trigger and that opportunities are being moved to the configured stage.
